TLC provides high-quality supports to individuals with I/DD and varying mental and physical health challenges. TLC’s mission is to help residents feel safe, supported and have opportunity to thrive while living a life they choose. 

 Summary of position:  A Life Enrichment Coordinators role is to plan, coordinate and facilitate activities that support Individual ISP goals which focus on Independence, Productivity and Social integration.   This position works with the area Program Directors, Financial Administrator, DSP and individuals to assure these goals are being met and documented per TLC and OAR standards.  This is a professional position where training and oversight for DSP’s may be needed.  It is expected that a Life Enrichment Coordinator focuses on positive forward progress, skill building and assisting individuals to accomplish their goals, while maintaining a professional standard of care. 

 Location: Gresham Area Homes – supporting 4 homes, up to 12 clients.  

Days/Hours:  Mon-Wed, up to 30-40 hours/wk. 

Supervisor: Executive Director   

 Duties:

-Participate in the ISP process, understand each individuals likes/dislikes, abilities and support needs. 

-Meet with each individual and their program director weekly to plan and facilitate activities in support of their goals.

-Pre-arrange funding for activities and assure proper documentation is completed for all financial supports provided.

-Create a stimulating and varying program of leisure activities, social events and productivity opportunities to assist residents in meeting their goals. 

-Reviewing Action plans and ISP goals, assuring goals are met and outcomes documented.   Daily entries to be completed in BEHCA for all resident goals supported. 

-Complete weekly summary reports for each resident noting goals accomplished, plan for upcoming activities and further supports identified to carry out future goals.

-Role model expected behavior and be a team player.  Work with behavior consultant to support PBSPs and guide staff on safe supports. To work as an active team member with your colleagues within the home to ensure the highest standards of care are achieved.

-To liaise and consult with staff and relatives regarding the care of residents and particular activities that may be beneficial and stimulating.

-To undertake any reasonable request which facilitates the resident’s right to choice, dignity and respect.

 

Requirements:

-Must be at least 21 years old and pass a criminal background check, then maintain criteria per OAR standards for providing care to Individuals with I/DD.  

-Must be familiar with and comply with all TLC policies and procedures. 

-Must be a licensed driver and maintain a clean driving record while employed by TLC.  Must meet criteria to be covered by TLC’s vehicle insurance. 

-Must have knowledge of the ISP Process and assure ISP training is completed for each person served. 

-Complete and maintain Core Competency training requirements needed for the position including, but not limited to: First aid/CPR, OIS, Confidentiality, mandatory abuse reporting, ISP and PBSP supports, Documentation/BEHCA training.
